How to Become an InMotion® Client

InMotion’s mission is to help people with Parkinson’s disease feel better every day. There is only one requirement for becoming an InMotion® client – a Parkinson’s disease diagnosis.

  1. Download / print and complete our Physician Approval Form (Section II must be completed by your physician).
  2. Once we receive the completed form from your physician, we will contact you to schedule a new client orientation session.
  3. Complete a new client orientation session.
  4. Schedule and complete an assessment, if eligible.
  5. Begin registering for classes. Follow-up meetings are available if you would like help understanding class recommendations.
